Ramos charged in to head Luka Modric’s free kick past Barca’s goalkeeper Marc-Andre ter Stegen, who got a hand on the ball but couldn’t keep it out.
The draw means Zinedine Zidane’s side will maintain their six points gap at the top of the table.
Barcelona captain Andres Iniesta, who missed ruled out for six weeks due to injury, was included in the squad and came in the 60th minute of the match.
Madrid also extended its unbeaten streak to 33 consecutive matches.
